The Additional Site Reporting form is used to report new physical locations that have been purchased/leased and contain programs that will be administered by the Health Authority/Member. For example, if a new program is created and requires additional leased space or another site leased due to expansion, HCPP must be kept apprised to ensure that appropriate property coverage is in place for the new site.
