CONSTRUCTION INSURANCE APPLICATION FORM
 

Risk Management Branch has arranged an owner-controlled course of construction and wrap-up liability insurance program (the Construction Insurance Program).  This form is used to make an application under the Construction Insurance Program.  You must submit the completed questionaire at least three weeks prior to the commencement of construction to allow HCPP time to place the project with insurers.  This form can be faxed to HCPP at 250-356-6222 or scanned and emailed to HCPP@gov.bc.ca.
